What is Onco-cardiology?

Which cancer patients are at a high risk for heart disease?
  • It includes patients undergoing:
    • Chemotherapy with anthracyclines drugs, which include: Epirubicin (Ellence), Idarubicin damycin), Valrubicin (Valstar)
    • Chemotherapy with some other drugs, which include mitoxantrone (Novantrone)
    • Certain targeted therapies include: Bevacizumab (Avastin), Trastuzumab (Herceptin), etc.
    • Chest radiation.

What are the different types of cardiac disorders in cancer therapy?
  • Cardiomyopathy and congestive heart failure (CHF): Heart damage-causing abnormality in the pumping of the heart
  • Myocarditis: Heartbeat affected by swelling of the heart.
  • Coronary artery disease: Blood vessels of the heart blocked
  • Arrhythmia: irregular heartbeat
  • Heart valve damage: the valves become either narrow and stiffen or leak
  • Abnormality of the outer membrane include:
  • Swelling (pericarditis)
  • Thickening or scarring (pericardial fibrosis) 
What steps should be followed in the diagnosis of cardiac conditions undergoing cancer therapy?
    • Physical examination to check heartbeats and sound of blood
    • Patients with a history of heart disease should undergo a complete cardiac profile (e.g. routine evaluation of cardiac enzymes through blood tests like troponin I and B-type natriuretic peptide (BNP) before initiation of chemotherapy along with an ECG and 2D-Echo.
    • Chest X-ray and angiogram are also advised if required.
    • Assessment of left ventricular function by 2D echocardiography
    • In cases of arrhythmias (abnormal heartbeats), appropriate documentation along with routine oncologic evaluation should be done.
How can these conditions be managed and treated?
      • Management includes the following:
      • Giving an alternative drug.
      • Reducing dosage or changing the route of administration of the drug.
      • Giving additional drugs which are cardio-protective
      • Reducing radiation therapy to the heart.
      • Treatment options include:
      • Medication:

      Use of angiotensin receptor blockers, angiotensin-converting enzyme inhibitors, or beta-blockers are considered to prevent cardiac disorders.
